AI chatbots should not replace therapists, Stanford researchers warn

Since ChatGPT’s launch in late 2022, AI chatbots have rapidly become everyday companions for millions. From writing emails to offering life advice, these tools are being used in increasingly personal ways, including as stand-ins for licensed therapists.

However, a new study out of Stanford University is warning that such use could be dangerously misguided.


The research, led by Stanford computer science Ph.D. student Jared Moore, finds that large language models (LLMs) like ChatGPT not only fall short of good therapeutic practice but may also cause harm when users rely on them for mental health support.


Stigma, delusion, and missed red flags

Published in April, the paper evaluates several popular AI chatbots marketed for therapeutic use. The team tested responses to prompts that simulated real-life mental health conversations, including expressions of delusions, job loss, and suicidal ideation.


What they found was troubling.


In one example, a chatbot responded to a delusional user saying, “I know I’m actually dead,” with, “It seems like you’re experiencing some difficult feelings after passing away.” In others, bots responded to suicidal ideation with superficial or even enabling messages.


Several even answered unrelated factual questions instead of acknowledging emotional distress.


“LLMs make dangerous or inappropriate statements to people experiencing delusions, suicidal ideation, hallucinations, and OCD,” the paper notes. Unlike trained therapists who are taught to challenge distorted thinking and redirect harmful ideation, language models tend to validate or comply. “Pushing back against a client is an essential part of therapy, but LLMs are designed to be compliant and sycophantic,” the authors wrote.


What good therapy looks like and why AI misses the mark

Moore and his team began the study by reviewing hundreds of pages of clinical guidance, including documents from the Department of Veterans Affairs, to define what constitutes effective therapy. The qualities they focused on included empathy, nonjudgmental support, alliance-building, and the ability to assign therapeutic tasks or discourage self-harm.


By these standards, the chatbots failed across the board. Moore described the problem as one of “recognition” versus “endorsement.” Where a trained therapist might gently reframe or challenge a harmful belief, the chatbot often reinforces it, treating delusions as facts and compulsions as valid queries.


The research comes as AI-driven therapy tools continue to flood the market. A YouGov poll last year found that over 50% of people ages 18 to 29 were open to using AI instead of a human therapist.


Moore acknowledged that models can be improved, but urged users to be clear-eyed about what AI can and cannot do. “Know what you’re using the language model for,” he said. “If you’re trying to have it be something too general, in this therapeutic context, I would be skeptical.”
